Both are primarily Vata disorders affecting the Asthi (bone) and Majja (marrow/nerve) Dhatus. The lumbar spine is the seat of Apana Vata — the downward-moving energy governing the lower body. When Apana Vata is disturbed (by prolonged sitting, heavy lifting, cold exposure, or constipation), it creates spasm, disc dehydration, and nerve compression. Ayurveda's approach is unique: rather than just treating the symptom, we restore the natural hydration and nutrition to spinal discs, release muscular spasm through targeted oleation, and correct the underlying Vata imbalance that caused the problem.
Kati Basti is the cornerstone therapy — a ring of warm black gram dough is placed over the lumbar region and filled with medicated oil (Ksheerabala Taila or Mahanarayan Taila) heated to body temperature. The oil penetrates deeply for 30-40 minutes, nourishing dehydrated discs, relaxing paraspinal muscles, and reducing nerve inflammation. Prushta Basti extends this along the entire spine. Agni Karma (thermal micro-cauterization at specific Marma points) provides immediate pain relief for acute sciatica by stimulating nerve regeneration. Basti therapy (Anuvasana with Dhanwantaram Taila + Niruha with Dashamoola Kwath) addresses the systemic Vata imbalance. Specific Yoga postures are prescribed for long-term spinal health.
Acute back pain responds rapidly — 60-70% relief within 5-7 Kati Basti sessions. Sciatica (Gridhrasi) typically requires 14 sessions for significant improvement. Disc bulge cases show measurable reduction in MRI findings after 21 days of intensive treatment in many patients.
